Pharmacist Menomonee Falls, WIPay From: $61.00 per hourMUST: Experienced pharmacist with 5 plus years of experience as a pharmacist. Must have an active Wisconsin pharmacist license. Must have inpatient experience. Must have PIC (Pharmacist in Charge) experience.DUTIES:Pharmacist will be responsible for, but not limited to the following:Review all patient charts for correct medication therapy and documentation; including patient drug allergies, drug/drug interactions with current medications, appropriate medication choice, route of administration and doseConduct weekly on-site visits to each facilityConduct monthly unit review of departmentAssist Hospital Administrator to ensure sufficient medication is in stock; including working with the pharmaceutical wholesaler to find alternatives, if necessaryReview and reconcile controlled substances logs and conduct an annual controlled substance inventoryAssist clinical staff with drug related informationCounsel patients on medication therapy, if neededPlan, monitor and evaluate drug programs and regimensQuadrant is an affirmative action/equal opportunity employer.
