About the Position
We are seeking a Nurse Practitioner to provide comprehensive care with a primary focus on chronic pain management in Grangeville, ID.
The provider will diagnose, treat, consult & follow up with patients while working collaboratively with a supervising physician and multidisciplinary clinical team.
This position emphasizes continuity of care, patient education, chronic disease management, quality improvement & a patient centered medical home approach.
• Diagnose & treat acute, chronic & long term healthcare conditions
• Primarily manage patients with chronic pain
• Conduct patient evaluations & develop appropriate treatment plans
• Provide follow up & continuity of care
• Address disease prevention & health promotion needs
• Educate patients & families regarding medical conditions, medications & preventive care
• Lead & collaborate with a multidisciplinary clinical team
• Coordinate care & community resources for patients with complex medical needs
• Participate in case management & quality improvement initiatives
• Maintain timely & accurate patient documentation in the EHR
• Support appropriate medical records & billing procedures
• Assist with development & revision of clinical procedures when needed
• Stay current with clinical knowledge through professional education & training
• Provide direction to clinical staff regarding patient care & communication
• Attend Medical Staff meetings
• Participate in Medical Staff committees & peer review
• Provide community education when requested
• Participate in performance improvement & quality initiatives
• Maintain EHR competency required for patient care
Requirements
• Current unrestricted Idaho Nurse Practitioner license
• Current BLS certification required
• Current unrestricted DEA certificate
• Controlled Substance Certificate from the Idaho Board of Pharmacy
• ACLS certification preferred
• Must maintain applicable medical staff privileges
• Strong clinical assessment & patient management skills
• Excellent communication & teamwork abilities
