Typical patients may include adults and older adults recovering from surgery or hospitalization, individuals managing chronic conditions, and those needing support to improve functional status and reduce fall risk. A typical day includes reviewing the schedule, driving to patient homes, completing assessments and treatments, updating documentation in the EMR system, and coordinating with other members of the home health team. You will perform gait training, transfer training, balance and strengthening programs, home exercise instruction, and home safety assessments. You may recommend adaptive equipment or environmental modifications to enhance patient safety and comfort. Caseloads are structured to allow for focused, patient-centered care, with time allotted for travel and documentation.
