Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the specific needs and challenges faced by patients and their families.

• Develop individualized treatment plans that are tailored to the capabilities and preferences of each patient.

• Provide therapeutic interventions, including individual therapy, family counseling, and group sessions, employing methodologies such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).

• Deliver crisis intervention services to aid patients in high-stress situations that require immediate support.

• Document patient interactions and treatment progress accurately in our Electronic Health Record (EHR) system, ensuring compliance with all regulatory standards.

• Collaborate with an interdisciplinary team of healthcare professionals, including medical staff, psychologists, and palliative care specialists, to ensure holistic patient care.

• Participate in weekly clinical meetings and contribute to case reviews, sharing insights and strategic input on complex cases.

• Remain up-to-date on the latest mental health trends, treatment modalities, and hospice care regulations through ongoing professional development.

Patient Population / Client Focus\n

The ideal candidate will work primarily with patients in a hospice setting, including those with terminal diagnoses. This role requires an understanding of the unique emotional and psychological needs of patients transitioning through end-of-life stages. You will provide support not just to the patients, but also to their families, helping them navigate anticipatory grief, decision-making, and the intricacies of bereavement.

Qualifications\n

• An active Licensed Clinical Social Worker (LCSW) license in California.

• A Master’s degree in Social Work (MSW) from an accredited institution.

• A minimum of 2 years of post-licensure clinical experience, preferably in the hospice or palliative care environment.

Compensation and Benefits\n
