Case Manager I: Discharge & Care Coordination

OSF HealthCare is seeking a Case Manager I who collaborates with the patient and family, physicians and a multidisciplinary team to ensure patient progression through the continuum of care and to independently develop a plan of care from admission through discharge with minimal direction.
