Lead Care Manager for Justice-Involved Reentry

Optima Medical Management Group is seeking a Justice-Involved Lead Care Manager (LCM) to provide comprehensive care management and community-based support for individuals transitioning from incarceration, in alignment with California DHCS CalAIM ECM guidelines.
