Work Environment\n

This position offers a remote work setting, allowing for flexibility to manage schedules while delivering essential services to our clients at the detox center. Typical work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed to address client needs. Flexibility in schedule is crucial to accommodate home visits and provide ongoing support to seniors and their families. You may also be required to be on-call to assist with urgent situations that arise outside of regular hours.

Key Responsibilities\n

• Facilitate effective communication among family members involved in senior clients’ care, helping resolve conflicts and ensuring family-centered strategies.

• Educate clients and their families on available community resources, support groups, and programs to promote social engagement and overall quality of life.

• Maintain accurate documentation of assessments, care plans, progress notes, and all relevant interactions within our electronic health record system.

• Stay informed about best practices in social work, including updates in policies and ethical standards to ensure compliance and quality of care.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n

We offer a competitive salary ranging from $60,000 to $100,000 per year, commensurate with experience.
