Schedule:Full-time, MondayFriday, 8:00 am 4:30 pm

Position Summary

The Medical Assistant Care Coordination supports providers and the Care Coordination team by managing clinical administrative tasks that promote continuity of patient care. Working from the corporate office, this position serves asa central pointof communication between patients, providers, pharmacies, insurance companies, and outside agencies. This role does not provide direct patient care in a clinic setting but plays a vital role in ensuringtimelycoordination of patient needs and provider support.

Duties/Responsibilities

• Manage and respond to patient portal messages, routing concerns to theappropriate providerwhen needed.

• Assistproviders with patient care coordination and follow-up activities.

• Coordinate communication with pharmaciesregardingprescription refill requests, medication questions, and prior authorizations.

• Supports clinic systems and processes that help standardizethe patientexperience.

• Complete and process FMLA, disability, Social Security, and other medical paperworkfor patientsaccurately and within required timelines.

• Coordinate with internal departments and external organizations tofacilitatepatient care.

• Maintainaccuratedocumentation within the electronic medical record (EMR).

• Maintain patient confidentiality andcomply withHIPAA and organizational policies.

• Perform other duties as assigned to support patient care and clinic operations.

Qualifications

• High school diploma or equivalent required.

• Current Medical Assistant certificationor degree

• Minimum of one year of experience in a medical office or care setting preferred.

• Experience using an electronic medical record (EMR) system.

• Strong organizational, communication, and time management skills.

• Ability to manage multiple priorities in a fast-paced environment whilemaintainingattention to detail.

• Excellent customer service and interpersonal skills.

Physical Requirements

• Ability to sit, stand, and walk throughout the workday.

• Frequent use of computers, telephones, and other office equipment.

• Ability to occasionallylift upto 25 pounds.

Work Environment

• This is a full-time, onsite position requiring attendance in the office five (5) days per week.

• Standard business hours with occasional flexibility based on operational needs.
