Job Description Summary / Objective East Side House (ESH) is a community resource serving the Bronx and Northern Manhattan. ESH’s Social Services programs align with our mission by providing high-quality programs and services through community-based programs and school partnerships. The Case Manager serves as a primary point of contact for individuals and families participating in the Housing Retention & Stabilization Services program. The Case Manager works collaboratively with participants to identify immediate and long-term needs, develop individualized service plans, and connect households to financial assistance, employment, education, public benefits, healthcare, and community-based resources. Case Managers work closely with Lead Social Workers and Social Workers when participants present complex psychosocial, behavioral health, crisis, safety, or family needs requiring specialized or higher-level social work assessment and intervention. Essential Functions and Responsibilities Assist participants with neighborhood orientation and connections to community resources. Conduct intake and needs assessments with assigned participants. Develop individualized service plans based on participant strengths, needs, priorities, and goals. Maintain consistent participant contact and monitor progress toward service-plan goals. Coordinate referrals and connections to public benefits, financial counseling, food resources, healthcare, employment, education, legal assistance, and other community-based supports. Assist participants with applications, documentation, appointments, and follow-up activities needed to access services and resources. Help participants strengthen self-advocacy, problem-solving, financial stability, and independent living skills. Identify participants whose needs require Social Worker or Lead Social Worker assessment or intervention and make timely internal referrals in accordance with established escalation protocols. Participate in case conferences and multidisciplinary service-planning meetings. Coordinate with Peer Mentors and community partners to strengthen participant engagement and access to resources. Maintain communication with internal and external service providers to promote continuity of services and reduce unnecessary duplication. Build respectful, professional, and trusting relationships with participants while maintaining appropriate professional boundaries. Use motivational interviewing and strengths-based engagement strategies to encourage participation and progress toward identified goals. Advocate for participants while supporting increased independence, informed decision-making, and self-determination. Maintain timely, accurate, confidential, and professional case records. Document assessments, service plans, participant contacts, interventions, referrals, outcomes, and case progress in accordance with program and agency requirements. Participate in supervision, case conferences, staff meetings, training, and quality-improvement activities. Support program data collection and reporting requirements. Maintain compliance with organizational policies, confidentiality requirements, applicable contractual and funder requirements, and established professional standards. Qualifications Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Public Administration, or a related field preferred. Relevant case management or human services experience may be considered in combination with education and other applicable qualifications. Experience working with individuals or families experiencing homelessness, housing instability, economic hardship, or other barriers to stability preferred. Knowledge of housing stabilization, public benefits, community resources, and New York City social services systems preferred. Strong organizational, participant engagement, advocacy, problem-solving, communication, and documentation skills. Ability to manage multiple participant needs, priorities, and deadlines while maintaining timely and accurate records. Ability to work collaboratively with multidisciplinary teams, community partners, and service providers. Expected Work Schedule The expected work schedule for this position is 9:00 a.m.-5:00 p.m., Monday through Friday, as aligned with program and service needs.
