MEDICAL SOCIAL WORKER

FIELD REGISTERED NURSE

Coordinates and supervises patient care to ensure continuity when patients are referred from referral sources. May be responsible for case management and provides skilled nursing services to patients in their homes. Responsible for recording clinical and progress notes. Works under the direct supervision of the Director of Clinical Services. Requirements: Registered Nurse with one year of acute care clinical experience within the last three years in a home health agency, home care setting, or healthcare facility. Meets California educational experience requirements. Minimum one year of infusion therapy experience; two years preferred. Ability to work flexible hours. Must be currently licensed in California.

HOME HEALTH AID

Provides personal care services and promotes comfort and independence for acutely and/or chronically ill patients in their homes. Records clinical and progress notes. Works under the direction of the Registered Nurse. Requirements: Completion of a state‑approved Home Health Aid training program and/or successful passing of a competency evaluation program meeting state and federal guidelines.

LICENSED VOCATIONAL NURSE

Provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Records progress notes and coordinates patient care activities. Works under the direction of the Registered Nurse or designated supervisor. Requirements: Graduate of an approved school of vocational nursing; meets California educational experience requirements. Minimum one year of acute care experience preferred; previous home health experience preferred. Licensed in California.

MEDICAL SOCIAL WORKER

Provides social case work and social histories for acutely and/or chronically ill patients in their homes. Records clinical and progress notes and coordinates other patient care activities. Works under the direction of the Director of Clinical Services. Requirements: Graduate of an approved school of social work with a Master’s Degree or licensed as a Clinical Social Worker. Minimum one year of social work experience; previous home care experience preferred. Licensed in California.

OCCUPATIONAL THERAPIST

Directs, supervises, evaluates, and provides occupational therapy services to patients in their homes as prescribed by the attending physician. Provides skilled therapy care, records progress notes, and coordinates other patient care activities. Works under the supervision of the Director of Clinical Services. Requirements: Graduate of an occupational therapy school approved by the Council of Medical Education of the AMA and the American Association of Occupational Therapy. Minimum two years of acute care experience; previous home care experience preferred. Licensed in California and registered with the Occupational Therapy Association.

PHYSICAL THERAPIST

Directs, supervises, evaluates, and provides physical therapy services to patients in their homes as prescribed by the attending physician. Provides skilled therapy care, records progress notes, and coordinates other patient care activities. Works under the supervision of the Director of Clinical Services. Requirements: Graduate of a physical therapy school approved by the Council of Medical Education of the AMA and the American Physical Therapy Association or equivalent. Minimum two years of acute care experience; previous home care experience preferred. Licensed in California.

SPEECH THERAPIST

Directs, supervises, evaluates, and provides speech and language therapy services as prescribed by the attending physician. Provides skilled therapy care, records progress notes, and coordinates other patient care activities. Works under the supervision of the Director of Clinical Services. Requirements: Member of the American Speech–Language–Hearing Association with a certificate of clinical competence. Master’s degree in Speech Language Pathology or Audiology and one full year of supervised work experience. Minimum one year of acute care experience; previous home care experience preferred.
