Nurse Practitioner (Primary Care Med)

Job Description Position Summary: Signing Bonus $5,000.00 Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including physicians, nurse practitioners, physician assistants, medical assistants and in‑house support staff. Essential Duties and Responsibilities Provide primary‑care services in accordance with Eventus WholeHealth site‑specific protocols and consistent with the standard of care for the specialties of family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term care facility work. Review and sign MARs as requested by facilities, sign all relevant primary‑care orders while at the facility or within 24 hours if faxed/emailed (phone orders, DME orders, death certificates, plans of care, PT/OT orders, hospice orders, etc.). Review and address labs within 24 hours of receipt. Manage all narcotic prescriptions for facility patients in accordance with the Eventus WholeHealth Narcotic Rx Policy. Provide on‑call triage duties per the on‑call triage protocol; adapt to updated schedules as provided. All providers must adhere to the current agreed triage schedule. Complete documentation and submit billing records per published policies on charting and bill‑sheet submission; provide the facility with a hard copy of electronic documentation within 48 hours of service. Maintain open and positive communication with facility staff and administration; give verbal/written instruction or feedback on medications and other pertinent caregiver information. Meet with the administrator or designated care coordinator before the start of patient‑care visits and before leaving the facility each day. Adhere to Eventus WholeHealth standards, policies, procedures, legal and regulatory agency rules, and professional ethical standards. Maintain patient confidentiality at all times, including proper use of cell phones, e‑mail, text messaging, patient charts, and EMR. Abide by HIPAA rules and regulations. Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding resident care. Qualifications/Education Master's degree in Nursing from an accredited institution and a current and valid state medical license. Graduated from an accredited physician‑assistant educational program. Passed the national certification examination for Physician Assistants. Licensed by the state in which you reside as a Physician Assistant.
