Seven Healthcare are seeking a dedicated RN specializing in Case Management for a travel assignment in Ashland, KY. This role involves working weekdays in a collaborative and supportive environment, focusing on the comprehensive assessment and management of high-risk members.

Key responsibilities of the RN – Case Management include:
- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers for individualized care plans
- Identify and address cost-efficient care levels
- Ensure compliance with treatment plans by arranging non-medical support
- Coordinate care transitions as needed
- Engage specialty resources to achieve optimal resolution
- Keep detailed records of outcomes during management
- Identify health promotion and illness prevention opportunities
- Act as patient advocate by safeguarding privacy and confidentiality
Requirements of the RN – Case Management include:
- KY RN License or Compact RN License
- HCHB experience
- 1 year home health experience
- Associates or Bachelor's Degree in Nursing or related field
- Case Management certification
