Street Medicine Peer Navigator — Empower Health & Housing Access
Healthcare in Action
Healthcare in Action in Los Angeles, CA is seeking a Peer Navigator to support unhoused individuals in navigating health and social services. You will accompany members to appointments, connect to programs, and help with plan of care, using shared experiences to build trust.
