Key Responsibilities\n

• Conduct thorough client assessments via telehealth or in-person when necessary, to develop and personalize comprehensive treatment plans targeting mental health needs.

• Provide targeted individual therapy sessions, along with couple and family therapy sessions, focusing on grief, loss, anxiety, and depression often encountered by hospice patients and their families.

• Work collaboratively with interdisciplinary teams, including physicians, nurses, and chaplains, to align care strategies and share insights regarding clients’ progress.

• Maintain accurate and up-to-date electronic health records (EHR), ensuring thorough documentation of client interactions, treatment plans, and updates on progress notes.

• Engage in regular professional development through supervision, workshops, and networking opportunities that foster skill enhancement and professional growth.

• Adhere to ethical and legal standards, ensuring that all client interactions are handled with utmost confidentiality and respect.

Patient Population / Client Focus\n

Your primary focus will be on clients in hospice care, who may grapple with complex emotional and psychological issues related to end-of-life experiences. Our patient population consists of individuals reflecting on their lives, family dynamics, and coping with anticipatory grief, for themselves or their loved ones. You will help them address their immediate mental health needs while providing a supportive and affirming presence during one of the most challenging times in their lives.

Qualifications\n

• Master's degree in Social Work from an accredited program, with strong foundational knowledge in clinical practices.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.

• A minimum of three years of professional experience in a mental health or outpatient care environment, demonstrating a solid track record of therapeutic effectiveness.

• Proficient in evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT), solution-focused therapy, and other relevant practices.

Compensation and Benefits\n

We offer a competitive salary range of USD 80,000 – 100,000 yearly, commensurate with experience.
