Registered Nurse, Care Command (Day Shift)

Job Summary The Access Care Manager (CM) is responsible for managing the entry and coordination of patients into the hospital. The CM addresses the planning, execution, and management of bed placement for all admissions from all entry points—including the emergency department, direct admits, and transfers—in accordance with clinical standards and guidelines of Phoebe Putney Memorial Hospital. The CM performs admission clinical reviews to determine appropriate level of care and patient status, and ensures patients are admitted to the most appropriate bed based on established criteria. All patient transfer activities comply with EMTALA regulations and guidelines. Qualifications Associate degree in Nursing required; Bachelor’s degree in Nursing preferred. Three years progressive clinical and basic management experience required. Registered Nurse (RN) with current Georgia license. Essential Functions Effective communication with care management, nursing, physicians, and other disciplines to discuss bed placement and admission status. Maintain patient confidentiality and follow hospital policies and work instructions. Demonstrate effective communication and team‑building skills; think outside the box in decision making. Apply knowledge of EMTALA regulations and regulatory/payer issues to admission and transfer processes. Perform utilization review (UR) functions, documenting and reviewing records, following up on outcomes, and ensuring timely, accurate documentation. Use clinical criteria for admission reviews, collaborate with physicians, and refer medical‑necessity issues to appropriate advisors. Provide timely notification to admitting physicians and follow up on authorization and liability issues. Engage in teamwork as both a team player and leader, educate staff and patients about the CM role, and participate in committees or projects. Identify and implement improvement actions to enhance patient flow, efficiency, and reduce readmissions. Attend staff meetings, mandatory in‑services, and competency evaluations on time; maintain current knowledge through education. Adhere to attendance and punctuality guidelines and departmental policies.
