General Purpose:

The Nurse Case Manager provides comprehensive case management, care coordination, assessment, education, and resource navigation to older adults and their family caregivers in the community. This role works collaboratively with members, families, caregivers, healthcare providers, and interdisciplinary teams to develop and coordinate person-centered care plans that promote wellness, independence, and quality of life.

Essential Duties and Responsibilities:

· Conducts in-home assessments, intakes, and reassessments to identify needs and determine appropriate services and level of care.

· Provides comprehensive case management, care coordination, and transition-of-care support.

· Coordinates services and communicates with healthcare providers, families, caregivers, and interdisciplinary teams.

· Develops, monitors, and updates individualized care plans based on changing member needs.

· Connects members and families with appropriate healthcare, community, and supportive resources.

· Provides education and guidance related to health, wellness, caregiving, aging, and long-term care planning.

· Monitors service delivery and participates in care conferences to ensure quality, continuity, and appropriate outcomes.

· Provides guidance, coaching, and support to direct-care staff.

· Maintains accurate and timely documentation in client records and completes required billing and quality-improvement documentation.

· Builds relationships with community organizations and supports outreach and program development.

· Responds to clinical concerns and provides on-call support as needed.

· Performs other duties as assigned.

Other Duties:

· Conducts employee supervisory visits to monitor competency levels of care provided by HHAs to clients.

· Provides on-call coverage after hours

· Responds promptly to all pages regarding all clinical care situations for members.

· Participates in Quality Improvement initiatives.

· Prepares and submits monthly documentation for billing.

· Maintains a record of acceptable attendance and punctuality.

· Observes all company rules and safety procedures.

· Other duties as assigned.

Qualifications

· Minimum 5 years of experience in case management, care coordination, and community-based care, preferably serving older adults.

· Strong assessment, critical-thinking, problem-solving, and organizational skills.

· Excellent communication and interpersonal skills with members, families, caregivers, providers, and interdisciplinary teams.

· Experience developing and managing individualized care plans.

· Demonstrated ability to navigate healthcare and community resources and coordinate multiple services.

· Supervisory, coaching, or team leadership experience preferred.

· Community or home-care experience preferred.

· Valid Ohio driver's license.

· Proficiency with electronic documentation systems and standard computer applications.

Requires good vision, hearing, speech and communication skills with the ability to ambulate, project voice, use the telephone, use the computer and read and write English. In addition, the employee must be able to stand on her/his feet for a period of up to eight (8) hours a day, move around the home without difficulty, transfer a 150-pound person and squat on the floor and rise to standing.
