The option of a flexible 40-hour workweek is possible within certain teams.Responsibilities:Provide high quality inpatient medical care for adult hospitalized patientsProvide direct clinical supervision and teaching on service to the Family Medicine Resident teamPerform comprehensive history and physical examinations, admissions, daily progress evaluations, and discharge planning along side the family medicine resident teamManage acute and chronic medical conditions in collaborationEnsure timely, evidence-based, cost-effective inpatient careCoordinate care transitions with outpatient providers, specialists, case management, social work, nursing, rehabilitation, and ancillary servicesMaintain electronic medical records efficiently and accurately, including timely completion of documentation, orders, discharge summaries, and medication reconciliationParticipate in interdisciplinary rounds and collaborate closely with nursing and hospital staff to optimize patient outcomes and patient experienceRespond to changes in patient condition and participate in rapid evaluation and escalation of care as appropriateParticipate in quality improvement, patient safety, and utilization management activities using a data-driven approachAttend required staff, provider, and hospital committee meetings except when clinical responsibilities prevent attendanceProvide patient care in a holistic manner including attention to physical, mental, emotional, social, and spiritual healthSupport safe discharge planning and reduction of avoidable readmissions through patient education and coordination of follow-up careMaintain electronic medical records efficiently and maximize available resourcesComplete required hospital documentation, billing, coding, and compliance activities accurately and timelyParticipate in quality improvement, patient safety, and regulatory readiness activitiesMaintain current certifications including all mandatory annual training, ACLS/BLS, and credentialing requirementsAdhere to professional code of ethics and compliance with hospital, state, federal, CMS, Joint Commission, and organizational requirementsCollaborate with interdisciplinary clinical and program teams across hospital and community settingsFoster respectful and collaborative relationships with physicians, advanced practice providers, nurses, consultants, support staff, patients, and familiesQualifications:Physician licensed to practice in New York State prior to start date of employmentBoard Certified as a Physician in Family Medicine or Internal MedicineExperience in hospital medicine, inpatient medicine, emergency medicine, critical care, or related field preferredCurrent BLS certification required; ACLS certification preferred or required within designated timeframeExperience working with low-income, diverse, multilingual populations highly desiredRate: $190,000 salaried.
