IDD Care Coordinator & PCP Specialist

RHA Health Services, LLC is seeking an IDD Qualified Professional (QP) for our ICF program. The QP will coordinate person-centered plans, lead the admission and census process, and ensure regulatory compliance while collaborating with the IDT to support independence and inclusion. The role requires a bachelor’s degree in a related field and experience with PCPs, ICF regulations, and Medicaid/HCBS rules.
