Duties Description • Determines and monitors the nutritional status for each individual assigned.
• Observes individuals at mealtime, which may involve travel to a variety of food service locations.
• Provides nutrition care instructions to individuals, families, family care providers, food and nutrition staff and other health care professionals.
• Maintains system for documenting information in an individual's medical record. Documents appropriate information on diet history, food intake, caloric and other nutrient requirements, drug-nutrient interaction, and existing disease states.
• Writes and approves menus and subsequent substitutions or changes.
• Provides discharge counseling to individuals/families.
• Utilizes appropriate computer support applications and maintains necessary databases to ensure appropriate care and service.
• Participates as a member of the interdisciplinary treatment team.
• Attends case conferences for the individuals assigned.
• Collects data and analyzes laboratory results and other pertinent information to determine nutritional status and determine the least restrictive and most reasonable dietary regiment for the individual.
• Makes recommendations to the team for the maintenance/improvement of nutritional status. May order diets or recommend changes to diet regimen to address nutritional concerns.
• Visits community-based homes to provide consultation on nutrition, menu planning, sanitation, food preparation, etc.
• Performs supervisory tasks as assigned by Nutritional Services Administrator 1 or 2.
• Requests supplies, materials and equipment.
• Conducts inspections of food preparation area to ensure compliance with State and Federal standards and to identify safety hazards.
• Presents new recipes and new products to food preparation staff.
• Supervises food service staff in the service of food in decentralized or congregate service areas.
• Plans and conducts in-service training activities for the food and nutrition program. Training may cover sanitation, safety, nutritional therapy, meal service, shopping, etc.
• Manages the provision of Clinical Nutritional Services.
• As assigned, plans menu.
• Supervises the standardization of recipes.
• Ensures the integration of cultural, religious and ethnic foods in the planning and delivery of balanced meals for culturally diverse patients.
• Determines meal acceptance, and initiates action to change menus to reflect preferences of the population served.
• Assists in the development of a quality assurance program for the nutrition program.
• Implements plans for correction action developed by the Nutritional Services Administrator 1 or 2.
• Documents information requested by Nutrition Services Administrator 1 or 2.
• May conduct peer review audits of Dietitians 1 and 2.
• Monitors food expenditures.
• Ensure current nutritional assessments and diet orders are accurately maintained within individual's electronic health record.
• Ensures that the appropriate adaptive dining equipment is being provided during meals.
• Actively communicates with the Interdisciplinary team(s).
• Travel required.
Under the supervision of the Nutrition Services Administrator 2, the candidate will:
• Coordinate medical records for clinical nutrition. Ensure medical records reflect nutritional status of the individuals and meet regulations.
• Maintain annual completion of assessments using the Nutrition Care Process.
• Attend ISP meetings, as well as entrance and exit meetings.
• Complete nutritional assessments, care plans, progress notes and discharge summaries.
• Makes recommendations to the treatment team for the maintenance and / or improvement of nutritional status.
• Recommend changes to meal plans to address nutritional concern.
• Process diet changes and provides the necessary training to the individual and staff concerning the change.
• Observes individuals at mealtime, which may involve travel to a variety of service locations.
• Provides nutrition care instructions to individuals, families, family care providers, food and nutrition staff and other health care providers in residential and Article 16 clinic setting.
• Work as part of an Interdisciplinary team to determine the nutritional needs of the individual.
• Schedule and Conduct Nutrition In-service training/ education for house staff. Maintain attendance records and record topics covered.
• Implement plans for corrective action.
• Ensure OPWDD regulations are adhered to and for overall health and safety of the individuals.
• Regular and proficient computer use. Knowledge and use of EHR system.
• May be assigned to or asked to assist in other assignments.

Minimum Qualifications Promotion:
Current registration with the CDR as an RD or certification as a Dietitian-Nutritionist by the NYSED, and one year of service as a Dietitian 1.

Open Competitive: Current registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD) or New York State Education Department (NYSED) certification in dietetics-nutrition and have completed a minimum of an additional 400 hours of supervised dietetics practice under the supervision of an RD.

Maintains current certification in New York State as a Certified Dietitian/Nutritionist.

Transfer: Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

• Please note that starting salary for this title reflects the Downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 18 ($66,951) and Downstate Adjustment ($4,000). Downstate Adjustment is also added to the job rate ($85,138).
