RN Case Manager Hospice

Job Summary:

The RN Case Manager - Hospice is responsible for providing skilled nursing care, pain management, and comfort to patients with terminal or chronic illnesses in a home hospice setting. This role involves developing, reviewing, and coordinating individualized care plans in collaboration with physicians, patients, families, and interdisciplinary teams. The RN Case Manager conducts ongoing assessments, documents patient progress, communicates significant changes, and educates patients and caregivers on symptom management and care techniques. The position requires a licensed Registered Nurse with a focus on hospice and palliative care, offering both independence and team collaboration. Responsibilities:

• Develop individualized care plans in collaboration with physicians, patients, and caregivers upon admission.
• Periodically review and recertify patient care plans as appropriate.
• Coordinate patient care through interdisciplinary communication and case conferences.
• Perform ongoing assessments of clinical, functional, and emotional patient status.
• Identify patient problems and monitor progress toward care goals.
• Communicate significant patient changes to physicians and team members.
• Document evaluations, patient responses, and progress toward goals accurately.
• Educate patients and caregivers on care techniques and end-of-life symptom management.

Required Skills & Certifications:

• Active Registered Nurse license in Vermont and New Hampshire or an unencumbered compact license.
• Graduate of an accredited school of nursing.
• Minimum of 2 years nursing experience, preferably in Medical/Surgical, Oncology, or Hospice care.
• Strong clinical assessment and case management skills.
• Excellent communication and interpersonal skills.
• Ability to collaborate effectively with interdisciplinary teams.

Preferred Skills & Certifications:

• Experience in Medical/Surgical or Oncology nursing.
• Previous hospice care experience.
