Work Environment

The role of the Family Nurse Practitioner will be performed onsite at our Assisted Living Facility, designed to accommodate the diverse needs of our clients. The work schedule is structured from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. In addition, participation in community outreach events during weekends is often expected, thereby allowing our nurses to engage directly with the community we serve. You will enjoy an average workweek of approximately 40 hours in a supportive, team-oriented environment that values collaboration and innovation.

Key Responsibilities

• Conduct thorough health assessments, including physical exams, health history intake, and screenings to determine patient needs.
• Diagnose and manage both acute and chronic medical conditions while emphasizing preventive care practices.
• Develop tailored care plans for patients and guide them through health promotion strategies aimed at improving overall wellness.
• Work closely with community organizations and stakeholders to create and implement impactful health education programs.
• Exercise cultural sensitivity in care delivery while advocating for patient rights and equitable access to healthcare resources.

Qualifications

• Must possess current certification as a Nurse Practitioner and maintain an active New York state nursing license.

Compensation and Benefits

This position offers a competitive salary range of USD 180,000 to 200,000 per year, depending on qualifications and experience.
