The Nurse Practitioner will work onsite at our outpatient office located in beautiful Poughkeepsie, New York. Key Responsibilities

• Conduct thorough health assessments, including physical exams and preventive screenings, tailored to a diverse patient population.
• Diagnose and manage both acute and chronic health conditions through evidence-based practices while promoting preventive care measures.
• Develop and implement individualized treatment plans in collaboration with multidisciplinary teams to optimize patient outcomes.
• Engage in culturally sensitive practices that advocate for patients' rights and access to comprehensive healthcare services.
• Coordinate referrals and ensure follow-up care, thus maintaining continuity of care and improving patient health trajectories.
• Utilize electronic health record (EHR) systems to maintain precise and up-to-date medical documentation in compliance with healthcare regulations.
• Participate in community outreach initiatives and health fairs, actively contributing to raising health awareness and providing education to underserved populations.
• Remain updated on current trends in internal medicine and relevant healthcare policies to continuously improve practice standards.

Patient Population / Client Focus

Your primary focus will be on patients from various backgrounds, many of whom may have limited access to healthcare services. A significant portion of our clientele consists of individuals facing mental health challenges, chronic conditions, and socio-economic hurdles. Qualifications

• Master's degree in Nursing from an accredited institution.
• Current and valid certification as a Nurse Practitioner.
• Active nursing license in New York State.
• Demonstrable experience of 3-5 years in clinical practice, preferably in community health or public health settings.
• Exceptional interpersonal and communication skills, with an emphasis on building rapport with diverse populations.

Compensation and Benefits
