Key Responsibilities

• Conduct thorough health assessments by obtaining complete medical histories and performing detailed physical examinations.
• Develop, prescribe, and adjust pharmacologic and non-pharmacologic treatment plans tailored to address both chronic and acute pain issues.
• Educate and counsel patients on pain management strategies, emphasizing the importance of adherence to treatment regimens and understanding medication side effects.
• Provide ongoing care and support for patients experiencing chronic pain, ensuring their treatment aligns with their individual goals and preferences.
• Utilize state-of-the-art tools to prescribe appropriate medications and proactively check prescription monitoring programs for patient safety.
• Work closely with physicians and other specialists to refine treatment strategies and deliver coordinated care.
• Utilize strong clinical assessment skills to accurately evaluate patients and implement effective diagnostic techniques.
• Create and oversee comprehensive pain management plans that prioritize a patient’s unique circumstances and conditions.
• Demonstrate excellent communication and interpersonal skills to build trusting relationships with patients and their families.

Patient Population / Client Focus

This position focuses primarily on residents within our Assisted Living Facility, which primarily serves an elderly population dealing with various chronic pain conditions. Your expertise will be critical in addressing the complexities surrounding age-related health issues, ensuring that every treatment plan is compassionate, effective, and respects the individual’s beliefs and wishes.

Qualifications

• A Master's Degree in Nursing (MSN) is required to ensure a strong theoretical and practical foundation in clinical practice.
• Possession of an active and valid Nurse Practitioner license in the state of New York.
• At least 2 years of professional experience in pain management or a related field is essential.
• Comprehensive knowledge of pain disorders and the ability to perform in-depth evaluations of patients with complex pain conditions.
• Proficiency in the appropriate prescribing of pain management medications and experience with diagnostic testing to identify pain sources.
• Skilled in performing procedures such as nerve blocks, spinal injections, and managing pain pumps.

Preferred Experience

• Experience working in a geriatric setting, particularly within assisted living or rehabilitation facilities.
• Previous involvement in interdisciplinary teams focused on enhancing patient care outcomes.
• Demonstrated ability to engage in shared decision-making with patients and their families regarding treatment options.

Compensation and Benefits
