Key Responsibilities

In your capacity as an Internal Medicine Nurse Practitioner, your responsibilities will include:

• Conducting thorough initial patient histories and comprehensive physical examinations to assess health status.
• Developing and documenting personalized care plans, communicating them effectively to patients and their families, and ensuring necessary screenings and preparations are completed prior to any interventions.
• Performing daily patient rounds in the comfort of their homes, monitoring their health, and adjusting care plans as needed.
• Ordering medications, laboratory tests, and diagnostic procedures that align with patients' needs and documentation protocols.
• Interpreting complex laboratory results and patient data to guide treatment decisions with precision.
• Collaborating closely with attending physicians, other nursing staff, and allied healthcare providers to ensure cohesive care and optimum health outcomes for each patient.
• Writing detailed discharge summaries, prescribing necessary medications, and making referrals for further care when necessary.
• Educating patients and their families about necessary post-discharge care instructions to promote recovery and well-being.
• Engaging in continuous quality improvement initiatives, research, and educational activities to enhance personal and organizational performance.
• Executing additional duties related to patient care as required, ensuring a comprehensive approach to health management.
