Key Responsibilities

• Conduct thorough assessments and evaluations of patients during house calls, including physical exams and health histories.
• Develop, implement, and manage comprehensive, individualized healthcare plans, focusing on both immediate and long-term patient goals.
• Utilize established protocols to diagnose and treat acute and chronic illnesses, ensuring the highest standard of care.
• Educate patients and their families on health maintenance, prevention strategies, and lifestyle modifications.
• Maintain accurate and up-to-date health records using electronic health record (EHR) systems to support continuous care and facilitate team collaboration.

Qualifications

• Master's Degree in Nursing from an accredited program.

Compensation and Benefits
