Key Responsibilities

• Collaborate with a team of healthcare professionals to develop, implement, and evaluate comprehensive care plans, adhering to institutional policies and regulatory standards.
• Conduct thorough and detailed assessments, including surgical, medical, and psychosocial health histories. Document pertinent findings and maintain an accurate problem/medication list.
• Order and interpret diagnostic tests, including laboratory and imaging studies, to establish clinical impressions and manage patient care effectively.
• Provide education and counseling to patients and their families regarding illness processes, care management, and health promotion strategies, enhancing their ability to make informed decisions about their care.
• Engage in proactive follow-ups with patients to monitor progress and make necessary adjustments to care plans.

Patient Population / Client Focus

Our Patient Population includes individuals from diverse backgrounds and socioeconomic statuses. You will be providing care to patients with a wide variety of health concerns, with a particular focus on behavioral health and chronic illness management. Every interaction aims to empower patients, ensuring they have the resources and understanding necessary to manage their health effectively.

Qualifications

• Must be board-certified in Family Medicine or Internal Medicine.
• Possess an active, unrestricted state license to practice in New York.
• Hold a Master of Science in Nursing, a Doctor of Nursing Practice, or be a graduate from an accredited Nurse Practitioner Certificate Program.
• Authorized to work in the United States.
• Exhibit excellent customer service skills and a commitment to patient-centered care.
• Demonstrate flexibility and adaptability in a fast-paced clinical environment.

Preferred Experience

Prior experience in a primary care or behavioral health setting is highly desirable. Familiarity with electronic health record (EHR) systems and telehealth platforms is advantageous, as these tools are commonly used to streamline patient care and maintain accurate health records. A history of engaging with patients from various backgrounds will be beneficial in fostering a supportive therapeutic relationship.

Compensation and Benefits

This position offers a competitive salary range of $150,000 to $160,000 annually, commensurate with experience and caseload.
