Position Overview

Gundersen Health and Wellness Clinic is seeking a dedicated and skilled Nurse Practitioner (NP) to join our outpatient practice at the Center for Advanced Cardiac Care (CACC) in New Paltz, New York. This full-time position involves working on-site within a dynamic multidisciplinary team, focusing on the unique needs of patients within the cardiac specialty. In collaboration with the Chief Nurse Practitioner and the CACC Medical Director, you will play a crucial role in providing exceptional care and improving patient outcomes.

Key Responsibilities

As a Nurse Practitioner in our Cardiology Clinic, you will be responsible for:

• Obtaining comprehensive patient histories and conducting thorough physical examinations to assess cardiovascular health.
• Ordering and interpreting diagnostic tests such as EKGs, echocardiograms, and laboratory studies to accurately diagnose and manage chronic and acute cardiac conditions.
• Formulating and implementing personalized treatment plans that include health maintenance strategies and patient education.
• Providing counseling to patients and their caregivers regarding disease processes, management outside of the clinic, and adherence to treatment protocols.
• Coordinating hospital admissions as necessary and collaborating with inpatient teams to ensure safe transitions of care for patients.
• Documenting patient encounters and care details in the electronic medical record (EMR) to ensure continuity and quality of care.
• Participating in research initiatives to integrate the latest findings and practices into clinical care.

Patient Population / Client Focus

Your work will focus on a diverse patient population, including individuals with complex cardiac issues such as advanced heart failure, pulmonary arterial hypertension, and those transitioning post-heart transplant. Qualifications

To qualify for this role, candidates must possess:

• A Master's degree in Nursing with specialty training as a Nurse Practitioner focusing on adult care.
• Licensure as a Registered Professional Nurse and Nurse Practitioner in New York State.
• Board certification as a Nurse Practitioner.
• Current certifications including BCLS, ACLS, DEA, and NPI.
• Proficiency with electronic medical records systems and Microsoft Office applications.

Preferred Experience

Ideal candidates will have:

• 1-2 years of clinical experience in a hospital setting, preferably within cardiology.
• Experience managing advanced therapies and understanding complex cardiac care needs.
• Familiarity with providing education and support for heart failure, arterial hypertension, and transplant care.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $220,000 per year, commensurate with your experience and qualifications.
