Key Responsibilities

• Conduct thorough health assessments and comprehensive physical examinations for patients of all ages, including routine screenings and health evaluations.
• Order, perform, and interpret diagnostic tests such as lab work and imaging studies to inform clinical decision-making and treatment plans.
• Collaborate with the healthcare team to develop and implement individualized, patient-centered care plans that address both immediate and long-term health objectives.
• Prescribe medications and administer appropriate treatments based on clinical findings and patient needs, ensuring adherence to best practices.
• Educate and counsel patients and their families about health maintenance, preventive care, and management of chronic diseases, enhancing their understanding and engagement in the care process.
• Participate in regular team meetings and case conferences to review patient progress and continuously improve care coordination.
• Maintain detailed and accurate medical records using our electronic health record (EHR) system, ensuring compliance with all federal and state regulations.

• Contribute to quality improvement initiatives, participating in audits and performance evaluations aimed at elevating clinical outcomes.

Qualifications

• A valid and unrestricted Nurse Practitioner license in New York is required.
