Key Responsibilities

• Conduct thorough initial screenings, inclusive of comprehensive health histories and physical examinations, ensuring every patient’s health fundamentals are accurately documented.
• Manage and update patient medical records meticulously, verifying their accuracy during each appointment to ensure optimal care delivery.
• Interpret diagnostic tests and analyze patient data to identify health concerns, determining the appropriate follow-up care, and collaboration with other healthcare providers when necessary.
• Deliver outstanding patient care while upholding company productivity metrics, thus contributing to both patient satisfaction and practice efficiency.
• Act as a primary resource for patients seeking ongoing health guidance, providing counseling for chronic conditions and critical health issues, and ensuring clear communication throughout the treatment process.
• Educate patients on effective health maintenance strategies, disease prevention methods, and treatment options, with a particular emphasis on harm reduction techniques that improve overall health outcomes.

Patient Population / Client Focus

Your patient population will encompass a wide range of individuals, including families with children, young adults navigating health transitions, and older adults requiring specialized care management. Qualifications

• Current and valid Family Nurse Practitioner (FNP) license in New York.
• Completion of a Master’s degree program designed to prepare registered nurses to provide acute and primary healthcare services.
• Certification as a Family Nurse Practitioner in Family Medicine.
• A minimum of two years of nursing experience in a relevant specialty, showcasing your capability in providing high-quality care and patient management.

Compensation and Benefits
