Work Environment

This position is based onsite, primarily focusing on home care for diverse patient populations in Poughkeepsie. As part of our team, you will work alongside other healthcare professionals in a supportive and collaborative environment where your contributions to patient care are valued. Your typical work schedule will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events and health fairs may be required on weekends.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, ensuring individualized care plans are developed and executed.
• Diagnose and manage a range of acute and chronic medical conditions, focusing on preventive care and overall health promotion.
• Collaborate with local community organizations to devise and implement impactful health education programs tailored to community needs.
• Provide culturally competent care, ensuring all patients’ rights are respected and advocating for their access to necessary healthcare services.
• Coordinate patient referrals and follow-up care, ensuring continuity and quality of care is maintained throughout the healthcare process.
• Maintain accurate medical records that comply with regulatory standards, enabling seamless information sharing and coordination with other healthcare providers.
• Engage in community outreach activities, including health fairs, providing essential health screenings and education to local residents.
• Stay abreast of community health trends, evidence-based practices, and changes in relevant policies to inform practice and enhance patient care.

Patient Population / Client Focus

Your role will primarily involve working with underserved populations in the Poughkeepsie area. You will encounter patients from diverse backgrounds, including those with chronic health conditions, individuals seeking preventive care, and families in need of holistic health solutions. Your ability to connect with these communities and provide tailored support will be crucial in enhancing their overall health outcomes.

Qualifications

• Current certification as a Nurse Practitioner.
• An active New York state nursing license.
• A Master's degree in Nursing from an accredited institution.
• Strong interpersonal and communication skills, with the ability to engage effectively with diverse populations.

Preferred Experience

Compensation and Benefits

SLI Rehab & Physical Therapy offers a competitive annual salary ranging from $180,000 to $200,000, commensurate with your experience and qualifications.
