Primary Care Nurse Practitioner (Full-Time, Onsite)

Position Overview

Complete Care Healthcare Services, LLC is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team in Poughkeepsie, New York. This full-time, onsite position offers a competitive salary range of USD 150,000 to 160,000 annually, dependent on caseload, and allows the successful candidate to make a meaningful impact on the health and wellness of our diverse patient population.

Work Environment

The Nurse Practitioner will work primarily in a home care setting, visiting patients in their residences throughout Poughkeepsie and the surrounding areas. Key Responsibilities

• Conduct comprehensive physical examinations and obtain detailed medical histories from patients in their homes.
• Develop and implement evidence-based treatment plans utilising clinical guidelines and professional standards to address both acute and chronic health conditions.
• Diagnose and manage common acute illnesses and injuries while providing patient education for chronic disease management.
• Prescribe medications responsibly, considering their safety, efficacy, and cost, in compliance with state regulations.
• Provide counseling and recommendations for lifestyle modifications to promote long-term health and prevent illness.
• Facilitate care coordination and communication with other healthcare professionals as part of a multidisciplinary team approach.
• Document all patient interactions accurately and maintain up-to-date patient records in compliance with healthcare regulations and standards.

Patient Population / Client Focus

As a Nurse Practitioner at Complete Care, you will serve a diverse population, including children, adults, and elderly patients, many of whom may have complex medical needs. Your role will involve building strong therapeutic relationships with patients, addressing their physical, emotional, and social health challenges, and ensuring they receive holistic care tailored to their individual circumstances.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• Current and unrestricted Nurse Practitioner license in the state of New York.
• National certification from a recognized certifying body, such as the AANP or ANCC, is required.
• Must possess valid US work authorization.
• Strong interpersonal and communication skills to build rapport with patients and collaborate with healthcare team members.
• A commitment to providing high-quality patient-centered care.

Preferred Experience
