Key Responsibilities

• Conduct thorough assessments of patients, utilizing comprehensive health histories and both focused and complete physical examinations to inform clinical decision-making.
• Develop and initiate treatment and diagnostic plans in accordance with cardiology best practices, including the ordering of laboratory tests, imaging studies, and other diagnostic procedures.
• Prescribe appropriate medications and therapeutic interventions, ensuring adherence to established guidelines and protocols to optimize health outcomes for patients with cardiac conditions.
• Provide education and counseling to patients and their caregivers regarding the management of chronic and acute heart-related diseases, emphasizing prevention and health promotion tailored to individual needs.
• Collaborate effectively with physicians, other healthcare providers, and interdisciplinary teams to ensure a seamless and integrated patient care experience.
• Maintain accurate patient records in compliance with legal requirements and organizational policies, ensuring data integrity and confidentiality.

Patient Population / Client Focus

This position will primarily focus on managing patients residing in our assisted living facility, many of whom are dealing with various stages of cardiovascular disease or conditions that impact cardiac health. Qualifications

• A Master’s Degree in Nursing, with a specialization or focus on family or adult care is required.
• Certification as a Nurse Practitioner with a specialization in cardiology is preferred.

• Proficient in electronic health records systems, with excellent documentation practices to support clinical care.

Preferred Experience

• At least two years of clinical experience as a Nurse Practitioner in a cardiology or relevant medical setting.
