Work Environment

This position is based onsite at our assisted living facility, where you will work closely with a diverse team of healthcare providers. Key Responsibilities

• Conduct thorough health assessments and screenings for residents, ensuring an understanding of both acute and chronic health conditions.
• Develop and manage individualized care plans that encompass preventive care, diagnosis, treatment, and follow-up for a diverse patient population.
• Collaborate with community organizations and stakeholders to design and implement effective health education programs that cater to the community's needs.
• Advocate for patients' rights and equitable access to health services, providing culturally sensitive care tailored to meet individual needs.
• Coordinate referrals and follow-up care to maintain continuity in the treatment and management of patients’ health conditions.
• Document all patient interactions, assessments, and treatment plans meticulously to comply with healthcare regulations and standards.
• Participate actively in community outreach events and health fairs, offering health screenings and education to raise community awareness.
• Stay informed about current community health trends, innovative practices, and relevant state and federal policies.

Patient Population / Client Focus

As a Family Nurse Practitioner at Bright Health Medical Center, you will primarily serve a diverse population, including elderly residents in our assisted living facility. Qualifications

• Valid Nurse Practitioner certification.
• Active nursing license in New York State.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 180,000 to 200,000, commensurate with experience and qualifications.
