Work Environment

This role is performed onsite within local school facilities, providing a unique opportunity to engage directly with students and parents in a familiar environment. You will be part of a collaborative team, including educators, school staff, and community health workers, to develop and implement health programs that address the specific needs of the student population. The working hours generally stretch from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., and participation in community outreach during weekends may be required.

Key Responsibilities

As a Family Nurse Practitioner, your responsibilities will include:

• Conducting thorough health assessments and screenings for students, identifying both immediate health issues and long-term health risks.
• Diagnosing and managing a range of acute and chronic health conditions, focusing on preventive care and health promotion strategies to enhance overall well-being.
• Collaborating with school personnel and community organizations to develop health education initiatives that empower students and families.
• Advocating for patient rights and ensuring access to necessary healthcare services, particularly for underserved populations.
• Coordinating referrals and ensuring continuity of care, facilitating connections to specialist services when required.
• Maintaining accurate and detailed medical records that comply with all regulatory requirements, utilizing electronic health record (EHR) systems effectively.
• Participating in community outreach events and health fairs to provide health screenings and increase awareness about health resources available within the community.

Patient Population / Client Focus

Qualifications

The ideal candidate will possess:

• A current and valid Nurse Practitioner certification.
• An active state nursing license in New York.
• A Master’s degree in nursing from an accredited program.
• A minimum of 3-5 years of clinical experience, particularly in community health or public health sectors.
• Exceptional interpersonal and communication skills, enabling effective interaction with diverse populations.

Preferred Experience

Compensation and Benefits
