Key Responsibilities

• Conduct comprehensive patient assessments, including physical examinations, medical histories, and diagnostic testing to develop individualized care plans.
• Interpret and implement the philosophies and objectives of Comfort of Care and the Nursing Department, ensuring all patient rights and obligations are upheld.
• Provide direct patient care, which may include prescribing medications, performing clinical procedures, and managing acute and chronic conditions.
• Collaborate with interdisciplinary health team members to formulate and implement comprehensive patient care programs tailored to specific needs.
• Mentor and guide allied nursing personnel and other staff in delivering high-quality patient care while fostering their professional development.
• Engage in continuing education and professional development activities to maintain the latest knowledge in patient care practices and advancements in the field.
• Document all patient interactions and care decisions accurately, adhering to legal and ethical standards.
• Participate in departmental meetings and contribute to quality improvement initiatives aimed at enhancing patient safety and outcomes.

Patient Population / Client Focus

You will be providing care to a diverse group of patients, including those with complex medical needs as well as preventive care for chronic illness management. The patient clientele may include individuals across various age groups and backgrounds, requiring a tailored approach to ensure all cultural and individual needs are met effectively. This role is integral in bridging gaps in care and ensuring patient satisfaction and alignment with best practices.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Program.
• National certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) is required.
• Must hold an active, unrestricted New York state nursing license.
• Eligibility to work in the United States without restrictions.
• Proficiency in the use of electronic health records (EHR) and other clinical software tools.

Compensation and Benefits

The role offers a competitive annual salary ranging from USD 150,000 to 160,000 , commensurate with experience and caseload responsibilities.
