Work Environment

This position is primarily onsite at a local school in Poughkeepsie, where you will engage directly with students and their families. The work environment is dynamic and community-focused, creating a vibrant atmosphere conducive to health education and hands-on care delivery. Your work schedule will typically range from 9:00 a.m. to 6:00 p.m., with the flexibility to accommodate evening clinics until 8:00 p.m. and participation in community outreach during weekends as needed. An average workweek will total approximately 40 hours.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for students and their families to identify health needs.
• Diagnose and manage both acute and chronic medical conditions, providing preventive care and health promotion advice tailored to diverse backgrounds.
• Collaborate with community organizations and stakeholders to design, implement, and evaluate effective health education programs that resonate with the community.
• Advocate for patient rights, ensuring that culturally sensitive care is delivered and that patients have access to necessary healthcare services.
• Coordinate referrals and follow-up care to maintain continuity of treatment and positive health outcomes for all patients.
• Maintain accurate and detailed medical records that comply with healthcare regulations and organizational standards.
• Actively participate in community outreach events, health fairs, and wellness initiatives to raise awareness about health issues impacting the community.
• Stay informed on current community health trends, evidence-based practices, and relevant policies to enhance service delivery.

Patient Population / Client Focus

The primary focus of this role will be the student population at local schools in Poughkeepsie, as well as their families. You will be engaged with diverse community members, facing various health disparities. The ideal candidate will demonstrate a commitment to understanding and addressing these unique needs, working to empower individuals and promote healthy lifestyle choices in a school setting.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized bodies such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Active DEA# and NPI# numbers are required.
• Strong clinical assessment and diagnostic skills with an emphasis on community health.

Preferred Experience

• A minimum of two years of relevant clinical experience, ideally with a focus on primary care or community health settings.
• Experience in delivering healthcare in school settings or similar environments is preferred.
• Proficient in using electronic health records (EHR) systems for documentation and tracking patient care quality.
• Excellent communication and interpersonal skills, enabling effective collaboration with a diverse range of stakeholders.

Compensation and Benefits

The salary for this full-time position ranges from USD 150,000 to 160,000 annually, commensurate with experience and qualifications. In addition to a competitive salary, candidates will have the potential to receive a sign-on bonus of up to $10,000.
