Work Environment

This role is primarily onsite at our assisted living facility. Key Responsibilities

• Conduct thorough assessments of patients, including medical histories and physical examinations, to create individualized care plans.
• Utilize established protocols to diagnose and manage a variety of health conditions while ensuring patient comfort and safety.
• Educate residents and families on health management strategies, prevention of illnesses, and health promotion through workshops and consultations.
• Collaborate with other healthcare professionals to facilitate optimal patient care; attend and contribute to team meetings discussing patient progress.
• Maintain accurate and up-to-date health records in accordance with HIPAA regulations and organizational policies.
• Respond to patient inquiries and concerns promptly and effectively, demonstrating empathy and understanding.

Patient Population / Client Focus

You will primarily serve elderly residents living within the assisted living facility who may have complex medical conditions requiring coordinated care. The population includes individuals with chronic illnesses, mobility limitations, and cognitive impairments, necessitating a compassionate approach to healthcare interventions that promote autonomy and improve quality of life.

Qualifications

• Master's degree in Nursing from an accredited nursing program.
• Current certification as a Nurse Practitioner, demonstrating your qualifications to provide high-level patient care.

Compensation and Benefits
