Work Environment

The Nurse Practitioner will work onsite at our outpatient office in Poughkeepsie, a picturesque city known for its vibrant culture and community spirit. Key Responsibilities

• Conduct thorough initial patient histories and perform comprehensive physical examinations to establish a clear understanding of patient needs.
• Develop, document, and communicate individualized plans of care, ensuring that all necessary screenings and patient preparations are completed effectively.
• Perform daily patient rounds, monitoring progress and adjusting treatment plans as necessary, while ensuring excellent patient engagement.
• Write orders for medications, laboratory work, and necessary diagnostic tests, related to the care and treatment of patients.
• Analyze and interpret laboratory and diagnostic test results, providing insights and recommendations to enhance patient care.
• Engage with attending physicians, residents, nursing staff, and other healthcare providers to coordinate care and optimize outcomes.
• Prepare and document comprehensive discharge summaries, prescriptions, and referrals upon patient discharge, while ensuring patients understand their post-discharge care instructions.
• Attend to any related duties that may arise in a dynamic clinical environment, fully contributing to team efforts and maintaining compliance with all health regulations and standards.

Patient Population / Client Focus

This role primarily focuses on adult patients requiring internal medicine services. The Nurse Practitioner will be engaged with a diverse population that may present a variety of complex health issues. Your expertise will aid in diagnosing and managing conditions, providing critical education, and guiding patients through their healthcare journeys, thereby significantly impacting their health and quality of life.

Qualifications

• A Master’s Degree from an accredited Nurse Practitioner Program is required.
• National Board Certification in the relevant specialty area is mandatory for the role.
• Candidates must possess a current Nurse Practitioner license and Registered Professional Nurse license to practice in New York State.
• A collaborative practice agreement with designated protocols must be filed with the New York State Department of Education or a collaborative relationship attestation as per the Nurse Practitioner Modernization Act of 2015.
• Current Basic Life Support (BLS) certification is essential.

Preferred Experience

While not mandatory, experience in outpatient internal medicine or a similar healthcare setting is highly valued. Compensation and Benefits

The annual salary for this position ranges from USD 150,000 to 160,000, commensurate with experience and qualifications.
