Work Environment

This position is based onsite at our assisted living facility, where you will collaborate closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, and social workers. Key Responsibilities

• Conduct thorough assessments by obtaining detailed medical, physical, and psycho-social histories, leading to complete health evaluations for residents.
• Perform comprehensive physical exams, including age and history-appropriate assessments, to identify health needs and potential issues.
• Implement non-invasive preventive screening procedures, ensuring that residents receive timely and appropriate health checks.
• Formulate accurate differential diagnoses based on patient histories, physical examinations, and clinical findings to address a variety of health concerns.
• Develop personalized education plans for patients and their families, emphasizing health literacy and promoting self-management of medical conditions.
• Prescribe and administer pharmacologic and non-pharmacologic therapies tailored to each patient's individual needs.
• Collaborate with the healthcare team in refining care plans and participating in patient case conferences to enhance the quality of care.
• Maintain accurate and up-to-date electronic health records (EHR) to ensure seamless communication among health professionals and to track patient progress effectively.

This role focuses on a diverse population of residents within the assisted living facility, which includes elderly patients with various chronic and acute medical conditions. You will work closely with individuals dealing with multiple health challenges, requiring a comprehensive approach to patient care. Qualifications

• A Master’s Degree in Nursing as a Nurse Practitioner from an accredited institution.
• An active Registered Nurse (RN) license in the State of New York.
• Strong foundational knowledge in Internal Medicine, with the ability to diagnose and treat a wide range of medical conditions.
• Familiarity with current medical technology, patient management systems, and electronic health records (EHR) software.
• Exceptional communication and interpersonal skills, with an emphasis on patient education and advocacy.
• Capability to work collaboratively within a healthcare team and contribute positively to team-based patient care.

Preferred Experience

A minimum of 3 to 4 years of relevant clinical experience as a Nurse Practitioner is required. Prior experience working in an assisted living facility or similar environments is highly desirable. Compensation and Benefits
