Key Responsibilities

• Deliver comprehensive healthcare services to patients with internal medicine needs, including routine check-ups and urgent care.
• Conduct thorough patient interviews, obtain and review medical histories, and perform detailed physical examinations.
• Develop and implement individualized patient management plans based on assessment findings and patient needs.
• Coordinate with physicians and specialists to ensure continuity of care and optimal patient outcomes.
• Utilize electronic health records (EHR) to document patient encounters and update treatment plans while maintaining compliance with healthcare regulations.
• Educate patients on health maintenance, prevention, and wellness strategies to promote a healthier lifestyle.
• Participate in team meetings and case discussions to share insights and improve patient care strategies.

Patient Population / Client Focus

You will be serving a diverse patient population in Wappingers Falls and surrounding areas, typically comprising adults and seniors who may have chronic health conditions requiring regular management. Qualifications

• Graduation from an accredited Nurse Practitioner or Physician Assistant program.
• A valid PA/NP license in the state of New York is required.
• Certification through the National Commission on Certification of Physician Assistants (NCCPA) for PAs or national certification for NPs is preferred.
• A Bachelor's Degree is mandatory.

Preferred Experience

Compensation and Benefits
