Work Environment

Your work environment will be diverse and dynamic, as you will serve patients in their homes and engage with the local community. The typical work schedule is from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. While the average workweek is about 40 hours, participation in community health fairs and outreach events during weekends may also be required. This direct patient engagement allows you to see the immediate impact of your care on the community while promoting health education and preventive measures.

Key Responsibilities

• Conduct comprehensive health assessments, screenings, and evaluations for patients of all ages in a variety of settings.
• Diagnose and manage a spectrum of acute and chronic medical conditions, while emphasizing preventive care techniques and health promotion strategies.
• Collaborate with local organizations and community stakeholders to design and implement community health education programs that raise awareness and address healthcare disparities.
• Advocate for patients' rights and equitable access to healthcare services, ensuring they receive culturally sensitive care that respects their diverse backgrounds.
• Coordinate and facilitate referrals, follow-up care, and communication between patients and specialists to ensure continuity of care.
• Maintain accurate, detailed medical records and documentation in compliance with regulatory standards, enabling effective patient management and continuity.
• Participate actively in health fairs and community outreach events to provide screenings and educational resources that address health issues prevalent in our community.

Qualifications

• Current certification as a Nurse Practitioner, with an active New York state nursing license.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 annually, based on experience and qualifications.
