Work Environment

This position is primarily onsite, allowing for hands-on interaction with patients in a concierge medicine setting. You will work in a collaborative and supportive atmosphere, where our team of healthcare providers and specialists come together to offer comprehensive care. With a schedule that typically runs from 9:00 a.m. to 6:00 p.m., there may be additional evening clinics or community outreach events, especially health fairs on weekends, to accommodate the varied needs of our patient population.

Key Responsibilities

• Conduct thorough health assessments for individuals and families, focusing on both acute and chronic conditions.
• Diagnose medical issues, develop treatment plans, and implement preventive care strategies tailored to each patient's needs.
• Engage in health promotion through innovative health education initiatives, designed and implemented in collaboration with community organizations.
• Advocate for patients' rights and ensure equitable access to healthcare services, particularly for marginalized populations.
• Coordinate and manage referrals and follow-up care, fostering continuity and consistency in health management.
• Maintain accurate and thorough medical records in adherence to healthcare regulatory standards while utilizing electronic health record (EHR) systems effectively.
• Participate actively in community outreach programs and health fairs, providing valuable health screenings and educational resources.
• Stay informed about local health trends and evidence-based best practices, using this knowledge to inform clinical decision-making.

Patient Population / Client Focus

Your practice will primarily serve families and individuals from New Windsor and the contiguous communities. Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license in the state of New York.
• A Bachelor's Degree in Nursing is required; a Master's degree is highly preferred.
• A minimum of 3 years of clinical experience, particularly within community health or public health settings.
• Exceptional interpersonal and communication skills, demonstrating an ability to connect with diverse patients and their families.
• A genuine commitment to health equity and advocating for underrepresented communities.

Preferred Experience

• Experience in a concierge medicine or similar model is beneficial.
• Previous involvement in community outreach or health education programs.

Compensation and Benefits
