Work Environment

This onsite role is situated in an Assisted Living Facility, where you will have the unique opportunity to engage with a diverse population of residents ranging in age and health status. Your typical work hours will be from 9:00 a.m. to 6:00 p.m., with some evenings extending to 8:00 p.m. for clinic availability. Additionally, you may participate in community outreach and health fairs during weekends, fostering deeper connections within the community.

Key Responsibilities

• Conduct comprehensive health assessments for residents, focusing on their physical, mental, and emotional well-being.
• Diagnose and manage both acute and chronic medical conditions while supporting the overall health promotion for individuals and families.
• Implement preventive care measures and develop individualized care plans through collaborative discussions with patients and their families.
• Coordinate with local community organizations to design and deliver health education initiatives tailored to the needs of the population served.
• Advocate for patient rights, providing culturally sensitive care that respects diverse backgrounds and personal circumstances.
• Ensure continuity of care by coordinating referrals, managing follow-up visits, and maintaining accurate medical records in compliance with healthcare regulations.
• Engage in community outreach by participating in events and providing necessary health screenings to promote awareness and access to services.
• Stay informed of emerging trends in community health, best practices, and relevant healthcare policies to enhance service delivery.

Patient Population / Client Focus

This position primarily focuses on vulnerable and underserved populations residing in New Windsor. Qualifications

• Current certification as a Family Nurse Practitioner (FNP).
• An active nursing license in New York State.
• A Master's degree in Nursing from an accredited institution.

• A minimum of 3-5 years of clinical experience as an FNP, preferably within community health or public health settings.

Compensation and Benefits
