SLI Rehab & Physical Therapy is seeking a dedicated and experienced Family Nurse Practitioner to join our healthcare team in Newburgh, New York. Work Environment

The Family Nurse Practitioner will work onsite at our facility in Newburgh, NY, an area rich in diversity and community spirit. The hospital setting offers a supportive and collaborative environment where healthcare professionals can share knowledge and experiences to improve patient care. The typical work schedule is Monday through Friday, from 9:00 a.m. to 6:00 p.m., with potential evening clinics until 8:00 p.m., as well as occasional participation in weekend community outreach events or health fairs. You will average approximately 40 hours per week, engaging in meaningful work that impacts community health.

Key Responsibilities

• Conduct thorough health assessments and screenings, evaluating both individual and family health needs through comprehensive patient interviews and examinations.
• Diagnose and manage acute and chronic medical conditions, creating tailored care plans that prioritize preventive care and health promotion strategies.
• Collaborate with local community organizations and stakeholders to develop and implement targeted health education programs that address the specific needs of the population.
• Provide culturally sensitive care, advocating for patient rights and facilitating access to healthcare services for vulnerable communities.
• Coordinate patient referrals and follow-up care, ensuring that patients receive appropriate and continuous care aligned with their health goals.
• Maintain accurate and detailed medical records that adhere to regulatory standards and best practices, utilizing electronic health record (EHR) systems.
• Engage in community outreach events and health fairs, conducting free health screenings and promoting health awareness initiatives.
• Stay informed about community health trends, evidence-based practices, and relevant policies to continually enhance the quality of care provided.

Qualifications

• Current certification as a Family Nurse Practitioner, in good standing.
• Active nursing license in the state of New York.
• Master's degree in nursing from an accredited program.
• Minimum of 3-5 years of clinical experience, particularly in community health or public health settings.
• Exceptional interpersonal and communication skills, with an emphasis on working effectively with diverse patient populations.
• Demonstrated commitment to social justice and health equity, with a focus on advocating for vulnerable communities.

Preferred Experience

• Experience in a healthcare role within a hospital or community health setting.
• Familiarity with electronic health record (EHR) systems and data management.

Compensation and Benefits

We offer a competitive salary ranging from USD 180,000 to 200,000 annually, commensurate with your experience and qualifications.
