Work Environment

This full-time position requires you to work onsite, primarily in home care settings, allowing you the flexibility to meet patients in their comfort zones. The working hours are typically from 9:00 a.m. to 6:00 p.m., although you may extend into the evening until 8:00 p.m. during certain clinic sessions. Participation in community outreach events, such as health fairs, is expected on occasion during weekends, enabling you to foster meaningful connections with the local community.

Key Responsibilities

• Conduct thorough and comprehensive health assessments for individuals and families, utilizing evidence-based practices to inform your diagnoses and care plans.
• Diagnose, treat, and manage a range of acute and chronic medical conditions, handling both preventive care and health promotion tailored to community needs.
• Engage with community organizations to design and implement impactful health education programs aimed at promoting preventive care and health awareness.
• Provide culturally competent care, ensuring your patients feel understood and supported throughout their healthcare journey while advocating for their rights.
• Coordinate referrals and follow-up treatments, striving for seamless continuity of care and enhanced patient compliance.
• Maintain accurate patient medical records in accordance with HIPAA regulations and best practices, ensuring continuity across all touchpoints of care.
• Stay abreast of emerging community health trends, actively integrating new findings into your practice to enhance care delivery.
• Participate actively in community outreach, which may include providing health screenings and educational resources during local events.

Qualifications

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 per year, contingent upon your experience and qualifications.
