Work Environment

This full-time position is based onsite, primarily focused on direct patient care in varying environments—including patient homes and other community locations. The typical work schedule spans from 9:00 a.m. to 6:00 p.m. with occasional evening hours until 8:00 p.m. and some weekend outreach commitments. Expect an average workload of approximately 40 hours per week in a supportive and dynamic team atmosphere.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, with a focus on identifying acute and chronic conditions.
• Design and implement individualized care plans, managing both preventive care and chronic disease management to enhance patient health outcomes.
• Collaborate effectively with local community organizations to create and deliver health education initiatives, workshops, and outreach programs tailored to community needs.
• Provide culturally competent care, ensuring equitable access to healthcare resources while advocating for patient rights within and outside our facilities.
• Manage patient referrals and follow-up care coordination, ensuring a seamless healthcare experience and enhancing continuity of care.
• Maintain accurate and compliant medical records in line with regulatory and organizational standards, utilizing electronic health record systems proficiently.
• Actively participate in community health fairs and outreach events aimed at raising awareness about health issues, conducting screenings, and providing resources to underserved populations.
• Stay engaged with healthcare trends, evidence-based practices, and new regulatory policies to continuously improve patient care.

Patient Population / Client Focus

Qualifications

• Possession of current certification as a Family Nurse Practitioner (FNP) and an active registered nursing license within the state of New York.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 180,000 to 200,000, commensurate with experience and qualifications.
