Work Environment

This position is based onsite at our Wallkill facility, which is integrated with a hospital setting, allowing for seamless access to a variety of healthcare resources. Our work schedule typically runs from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Weekend participation in community outreach events or health fairs might be required and provides a unique opportunity to engage directly with the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, utilizing evidence-based practices to evaluate immediate health needs as well as long-term health risks.
• Diagnose acute and chronic medical conditions, creating individualized management plans that prioritize preventive care and promote overall wellness.
• Work closely with community organizations to design and implement targeted health education programs, addressing specific health challenges in the area.
• Advocate for patients’ rights within the healthcare system, ensuring that all patients receive the services they need, irrespective of their background.
• Coordinate referrals and follow-up care processes to guarantee continuity and quality of care across different healthcare providers.
• Maintain comprehensive and precise medical records in compliance with all local, state, and federal regulations, utilizing electronic health record (EHR) systems to streamline patient data management.

Qualifications

• A current certification as a Family Nurse Practitioner.
