Work Environment

Located in Wallkill, NY, our practice operates onsite within an assisted living facility, creating a nurturing environment that fosters direct and personable interactions between healthcare providers and patients. As part of a collaborative care team, you will work closely with a physician, other nurse practitioners, physician assistants, medical assistants, and care coordinators—all dedicated to addressing the unique healthcare needs of older adults.

Key Responsibilities

• Deliver exceptional primary care, including conducting routine and acute patient visits.
• Administer annual wellness visits and health risk assessments with a holistic perspective on patient health.
• Engage in effective care coordination across specialists, testing facilities, and supportive agencies to ensure comprehensive patient care.
• Lead population health initiatives by implementing preventive care measures (e.g., ensuring eligible individuals receive critical screenings).
• Support the care team with telephone triages and proactive outreach to patients.
• Educate patients about their health conditions, treatment options, and individual care plans.
• Participate in promotional activities to enhance community awareness of primary care services.
• Conduct home visits for patients with limited mobility as required.
• Report directly to the Medical Director, collaborating closely with operational leadership to maintain high-quality care standards.

Patient Population / Client Focus

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from recognized certifying bodies (AANP or ANCC).
• Current, valid state Nurse Practitioner license without probationary measures.
• Active DEA license allowing for necessary prescribing.
• Clinical experience in primary care settings, with specific emphasis on internal medicine, geriatrics, or family medicine.
• Proficient understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).

Compensation and Benefits

The annual salary for this role ranges from USD 180,000 to 200,000, commensurate with experience and qualifications. Alongside a competitive salary, Pulse Point Internal Medicine offers a comprehensive benefits package, including:
