Advanced Cardiology PC is seeking dedicated and compassionate Nurse Practitioners to join our team in Newburgh, New York. This full-time position offers an opportunity to provide high-quality, value-based primary care to adults on Medicare, emphasizing long-term patient relationships and proactive health management. Work Environment

This position is based onsite at a school facility in Newburgh, fostering a unique setting that allows for easy access to families and children in the community. The work schedule is Monday through Friday, from 9:00 AM to 5:00 PM, providing a stable and predictable work-life balance. Our collaborative environment encourages open communication and teamwork, enabling our Nurse Practitioners to thrive within our care model.

Key Responsibilities

• Deliver exceptional primary care to patients, including comprehensive evaluations, diagnostics, and treatment plans.
• Conduct office visits for both routine check-ups and urgent care issues, providing timely and effective interventions.
• Administer annual wellness visits and health risk assessments, ensuring thorough documentation and a holistic approach to patient health.
• Coordinate care with a wide network of specialists, labs, and agencies to enhance patient treatment and ensure continuity of care.
• Lead population health initiatives, such as ensuring compliant breast cancer screenings for eligible women every two years.
• Assist the care team with phone triage, patient outreach, and follow-up care to maintain patient engagement and satisfaction.
• Educate patients about their health conditions, care plans, and treatment options to empower them in their health journey.
• Participate in promotional activities to support community engagement and service outreach.
• Conduct home visits as necessary to deliver care to patients with mobility challenges or complex care needs.
• Report to the Medical Director, closely collaborating with operational leadership for continuous improvement and care excellence.

Patient Population / Client Focus

Our primary focus is on older adults, particularly those enrolled in Medicare, who often face multiple chronic conditions. We are committed to providing tailored healthcare solutions to enhance their quality of life. By building strong patient-provider relationships, our Nurse Practitioners help patients understand their health conditions and actively participate in their care plans.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC required.
• Current, active state Nurse Practitioner license free of probationary status.
• Active DEA license.
• Previous experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and health risk assessments.
• A collaborative spirit and a passion for working as part of a healthcare team.

Preferred Experience

• Experience working with older adults and understanding the unique challenges they face.
• Ability to engage effectively with a collaborative team to improve patient outcomes.
• Experience with care coordination and population health initiatives.

Compensation and Benefits

We offer a competitive salary range of USD 120,000 – 160,000 annually, commensurate with experience.
