Key Responsibilities

• Conduct thorough and comprehensive patient assessments through detailed medical history intakes, physical examinations, and necessary diagnostic testing.
• Design and implement individualized treatment plans, working in conjunction with cardiologists and healthcare teams to optimize patient care solutions.
• Monitor and manage chronic cardiovascular conditions, including but not limited to hypertension, heart failure, and arrhythmias, ensuring patients receive the care they need to improve health outcomes.
• Educate patients and their families about lifestyle changes, medication management, and preventive measures crucial for maintaining heart health.
• Perform and interpret vital diagnostic procedures such as echocardiograms, EKGs, and stress tests to assist in accurate diagnosis and treatment planning.
• Prescribe medications adhering strictly to established clinical guidelines while ensuring that patients understand their treatment regimens.
• Engage in follow-up care by scheduling routine check-ups and monitoring patient progress, showcasing a commitment to ongoing support.
• Collaborate seamlessly with multidisciplinary teams to ensure integrated care that addresses all aspects of a patient's treatment.

Qualifications

• A Master’s degree in Nursing from an accredited institution is required for this role.

• At least 2 years of experience in cardiology or a closely related field is necessary for the successful execution of job duties.

Compensation and Benefits

Guardian Health Nurse Practitioners offers a competitive salary range of $150,000 to $160,000 annually, reflecting the importance of this role within our organization.
